How Do I Make Or Reschedule An Appointment?

Making or rescheduling an appointment is as simple as giving us a call. We can be reached during normal business hours at {tel}.

What Is Your Cancellation Policy?

We understand that sometimes things don’t work out the way had we planned. If you need to cancel your appointment, a minimum of one business day is requested to cancel a regular appointment in order to avoid a $50 cancellation fee.

For surgical procedures, we require that you call 3 days in advance to avoid a cancellation fee. Cancellation fees for surgical procedures are as follows:

Surgical or Procedural appointment $150
PRP appointment $350

Do You Take Walk-Ins Or Same-Day Appointments?

Although we cannot guarantee that we will have an appointment available the day you call, we will always try to accommodate your schedule. To see if we have any available appointments, call us at {tel}.

What Should I Bring To My Appointment?

When you arrive for your appointment, please bring the following items with you so that we can expedite the check-in process and ensure that everything continues to run smoothly.

  • Insurance cards
  • A picture ID (school ID, driver’s license, etc.)
  • Physician referral–if required by your insurance
  • Medical history
  • List of prescriptions
  • Office Co-pay
  • Patient forms

Which Insurance Plans Do You Accept?

At Dr. Dubow Dermatology, we accept most major PPO plans. However, we are out of network for Blue Cross individual and family plans, and we are unable to accept any HMO plans.

We accept the following insurance plans:

  • BLUE CROSS (Prudent Buyer)
  • BLUE SHIELD (group/commercial plans)

For cosmetic procedures that are not covered by insurance, such as Botox, chemical peels, laser treatments, and hair loss treatments, the cost of the procedure is the patient’s responsibility. For all billing questions, please call us at {tel}.

How Do I Pay My Bill?

There are a few simple ways to pay your bill.

  • You can pay by phone by calling the number on the bill you received in the mail
  • You can send your payment through the mail with the billing statement you received
  • You can bring your payment to our office during normal business hours
